Abstract

The choice of treatment in patients with oral malignancies depends on accurate pretreatment staging and particularly the detection of lymph node involvement. Therefore staging of the neck should be as accurate as possible. Objectives: the goal of this review study was to perform a literature review to access MRI performance to evaluate lymph node metastasis. Methods: MEDLINE, through PubMed, was searched for articles published up to 2018, restricted to English publications. The inclusion criteria encompassed original articles that analyzed or discussed the use of MRI images in the evaluation of squamous cell carcinomas of the oral cavity (SCCOC) and distant metastasis in humans. Results: A total of 30 studies were considered eligible for inclusion in this literature review and assembled for discussion. Conclusion: Previously the surgery, the MRI exam should be allied with de PET-CT for a more accurate detection of lymph node metastasis related to SCCOC, and these exams should be repeated the following first year. The MRI has a good resolution for soft tissues therefore corroborating for determination of the tumor size, that can be a predictive factor of metastasis. Since MRI can lead to misinterpretation of the images, histopathologic exams of the lymph nodes from elective neck treatment should be performed.
